I usually generate a 1d6,1d4 compound table for each session. I roll on this table at the start of every round. If players are proactive and lots of chaos is going on I might skip rolling. If players are directionless or not taking much action I might roll twice on the table to get the action going. The hazard table system was made by Necropraxis, another amazing blog for Game Masters.
